Job Summary
We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to provide high-quality care in a post-acute and long-term care setting. The LPN will collaborate with residents, families, and interdisciplinary care teams to create a safe, therapeutic, and supportive environment. This role involves both direct and indirect patient care across multiple units, each serving distinct resident populations.

Unit Descriptions
Memory Care Unit (31 beds)
Serves residents with Alzheimer's and dementia, requiring a strong ability to use both verbal and non-verbal communication, understand behavioral expressions, and provide person-centered care.
Transitional Rehabilitation Unit (42 beds)
Provides short-term skilled nursing and rehabilitation services for patients recovering from surgery, fractures, strokes, and other acute health conditions. Care frequently includes IV therapy, wound care, and geriatric assessment.
Long-Term Care Unit (42 beds)
Provides home-like long-term placement and ongoing daily care for residents requiring continuous support.
Responsibilities
Deliver safe, patient-centered care in accordance with state regulations and facility standards
Provide clinical support including medication administration, wound care, assessments, and documentation
Collaborate with care teams to promote optimal resident outcomes
Communicate effectively with residents, families, and care providers
Maintain flexibility to float across units as needed
Support 24 / 7 coverage with day or night shift availability
Requirements
Education
Graduate of an approved LPN program
Active and unrestricted LPN license (state license or compact license)
Experience
1 3 years of LPN experience preferred
Experience in LTC, SNF, or post-acute rehab settings highly valued
Skills
Strong teamwork and communication abilities
Understanding of medical terminology preferred
Ability to adapt to varying resident needs and unit demands
Ability to work rotating shifts and float to other units
Contract Details
13-week contract
Day (7a 7p) or Night (7p 7a) shifts
Holiday coverage required
Up to 16 hours of orientation may be unpaid
Up to 3 shifts may be canceled per 13-week contract
Candidate must reside outside a 50-mile radius
Certifications Required
BLS (AHA or equivalent)
