About The Position

Job Title : Registered Nurse

Experience : Minimum of 1 year of Adult Home Health experience or 1 year of Hospice experience is required.

License : Must have a nursing license in hand.

Certifications : CPR, BLS

Must-Have :

• Proficient in OASIS and have used it within the last 6 months, or must complete a retraining course.
• Proficient in HomeCare HomeBase and have used it within the last 6 months, or must complete a retraining course.
• Must be open to traveling up to 75 miles from the assigned care center.

Description :

Coordinates and directs the provision of home nursing care according to physician orders.

Follows agency policies and procedures through the competent application of the nursing process.

Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Travel required for home visits and periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average patient load is 5-10 patients per day, which is expected to equate to roughly 6-8 productivity points.

Documentation and completion of patient visits are essential within the set timeline.

No guaranteed hours are provided.

Must accept every patient on the schedule, regardless of the distance from previous appointments.

Weekly productivity points of 30 are required, equating to 40 hours of work each week.

On-call duties may vary by location, and weekend rotations may be required.

A valid vehicle is necessary for driving to patients' homes daily and for attending weekly meetings.

Local candidates may be accepted at the same conditions as travelers.

Candidates must agree to travel and the expected patient load and documentation requirements.
