Your Role as a Case Management Support Specialist

  • Coordinate and facilitate Team Conference meetings effectively.
  • Gather, maintain, and manage all Case Management documentation including referrals, admissions, orders, and schedules.
  • Communicate daily Case Management updates efficiently.
  • Assist with pre- and post-discharge tasks to ensure patients, families, and clinicians are well-informed and ready.
  • Compile Quality Improvement and department Performance Improvement data accurately.
  • Support the collection of Key Care Indicators and clinical outcomes data.
  • Contribute to regulatory and Joint Commission survey preparation efforts.
  • Qualifications

  • High school diploma or equivalent preferred.
  • Proficiency in Microsoft Office is essential.
  • Strong typing skills with both speed and accuracy.
  • Familiarity with medical terminology is preferred.
  • Experience with insurance / payor systems is a plus.
  • Good visual acuity and strong communication skills.
  • Flexibility to work weekends, evenings, or nights as necessary.
  • Availability to work on religious / legal holidays as scheduled.
