Description :
The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in their daily lives.
They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability.
The physical therapist will work with the individual to prevent the loss of mobility.
Essential functions and responsibilities include performing patient evaluations through the process of obtaining history, conducting relevant systems review, and selecting and administering specific tests and measures to obtain data.
The physical therapist establishes an individualized plan of care and selects treatment techniques that address the specific impairments identified in the evaluation and in accordance with physician orders.
They conduct ongoing reassessment of the patient and their response to treatment.
Initiating and managing the discharge plan will begin at the first visit and continue throughout treatment.
The physical therapist supervises and delegates to ancillary staff and students consistent with state laws and department policy.
They complete legible, accurate, and clear documentation within the established timeframes identified in the department policy and procedure and in compliance with accreditation requirements and insurance regulations.
Communication with the patient, family, caregivers, significant others, and members of the healthcare team is essential to promote maximum benefit of care utilizing AIDET.
The physical therapist assesses the educational needs of the patient, family, caregiver, or significant other and provides education to meet those needs.
Participation in professional growth and development is encouraged through educational programs, including in-service meetings, workshops, and reading current literature concerning the scope of practice.
The physical therapist actively participates in quality improvement activities.
Providing an environment conducive to safety for patients, visitors, and staff is imperative.
The physical therapist complies with appropriate and approved safety standards.
They function within the scope of practice as defined by the state's practice act.
The physical therapist performs other related duties as required and directed.
