Job descriptionCase Management position at BronxCare Hospital Center Expected to have knowledge of discharge planning and making referrals Will typically be working on a MS or Tele unit and will split the floor with a Social Worker Must be able to function independently on the unit and work collaboratively with an interdisciplinary team and patients for discharge planning Expected to round on the unit with physicians / team Caseload typically has 33-40 patients; Most floors are split with 1 CM & 1 SW; Caseload : 17-25 patients Need to be able to handle at least 20 patients Experience or 'Must have' skills : Must have 1 year of inpatient case management experience; Prefer prior critical care experience Required Certifications : BLS Prefer some knowledge of CarePort, MCG, and / or InterQual Prefer some Utilization Review experience Shifts M-F 8-4 : 30 (7.5 Hr shifts with a 1 hr lunch) Occasionally weekend coverage with manager approval Occasionally holiday coverage with manager approval Working on a MS or Tele unit Discharge planning role Common types of patients : Discharge to Home, Community, or SNF BLS, PRI (Patient Review Instrument) Certification, SCREEN Training Certification Department Specific Orientation : 2 days of Hospital Orientation- will include EMR training; 1-2 wks with a preceptor depending on how fast they can be checked off.
