Nurse Practitioner (Fayetteville)

Pride Health is hiring a Nurse Practitioner in Fayetteville, NC! (Contract)

Work between 2-3 Long Term Care settings near Fayetteville, NC

Schedule : Monday-Friday (8am-5pm)

Contract : 3 month contract to hire

Pay Rate : $66-70 / hour

Specific Duties :

• Provide specialized care to high-risk health plan members
• Work as part of an interdisciplinary team
• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate documentation, including ICD-10 coding
• Communicate and collaborate with the care team
• Facilitate advanced illness and care planning discussions
• Offer counseling and education to patients and caregivers
