  • Job Description
  • Administer skilled nursing care to patients in their place of residence as prescribed by the physician and according to the established plan of care.
  • Work under the supervision of a Registered Nurse (RN) and coordinate care with the interdisciplinary team and the patient / family.
  • Deliver care in compliance with State, Federal, and Medicare standards.
  • Assist the RN in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs.
  • Participate in planning, coordinating, and implementing care in conjunction with the RN.
  • Provide effective communication to patient / family, co-workers, and other health care professionals.
  • Assist the patient / family in learning appropriate self-care techniques.
  • Provide care utilizing infection control measures that protect both staff and patient.
  • Assist the RN and / or physician in performing specialized duties related to end-of-life care for Hospice patients.
  • Assist with admitting, discharging, and transferring of patients.
  • Possible on-call duties.
  • Participate in annual in-service and performance improvement programs.
  • Perform other related duties as assigned.
  • Critical Information
  • Current Nursing License in the Practicing State.
  • Valid Driver's License and current auto liability insurance coverage.
  • Ability to recognize and maintain professional boundaries in relationships.
  • Ability to recognize an unsafe working environment and notify a supervisor immediately.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
  • Education / Licenses Needed
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current CPR Certification.
  • Two years of experience as an LPN / LVN in a clinical setting, preferably in a home health or hospice setting.
  • Benefits :
