Social Worker

POSITION PURPOSE

The Social Worker position ensures safe, effective and patient-centered psychosocial and care management services for patients in a rural Critical Access Hospital. The position provides assessment, counseling and care coordination in alignment with American Case Management Association (ACMA) social work standards, New Mexico Board of Social Work (NMBSW) regulations, Det Norske Veritas (DNV) accreditation requirements and Centers for Medicare & Medicaid Services (CMS) Conditions of Participation for Critical Access Hospitals (CAHs). This position emphasizes the unique challenges of rural healthcare, ensuring equitable access to resources and support across the care continuum. MINIMUM MANDATORY qualifications

Experience :

• Master's Degree in Social Work from an accredited college or university.
