Job purpose

To oversee, lead and motivate Case Management staff in the performance of their daily duties.

  • Model exemplary behavior in the performance of all job tasks
  • Monitor, evaluate and make necessary changes which further enhance service delivery

Duties and responsibilities

  • Provide direct daily and weekly supervision to Case Management staff
  • Carry an active case load (size to be determined)
  • Conduct Pre-Screenings, Pre-Admission and Admission tasks for participants
  • Review and make final admission recommendations for with senior leadership
  • Schedule all VLC programing and manage weekly schedules utilizing staff and external partners
  • Review and approve all Track assignment recommendations made by Case Managers
  • Works closely with VLC staff and if authorized family & friends of residents
  • Evaluate the effectiveness of the Screening and Admissions process
  • Create and build relationships with referral agencies and other external partners
  • Regular (weekly) review of Case Management charting
  • Ability to counsel clients who may be undergoing a crisis
  • Strong up to date knowledge of referral agencies, VA services and entitlements
  • Manage Apricot database and subsequent reporting
  • Facilitate selected resident program groups
  • Demonstrated flexibility and adaptability in an environment subject to frequent programmatic changes
  • Conduct weekly Case Management team meetings
  • Develop and maintain a Quality Management program for all Case Management functions
  • Weekly updates and monthly reports on programmatic performance to the Executive Director and Program Manager
  • Strong computer fluency in MS Office and other databases utilized
  • Other duties as assigned by the VLC Program Manager or Executive Director

    • Qualifications

    Strong preference for three or more years’ prior case management experience working with Veterans in North Carolina or an understanding of the culture and service availability for Veterans

    Experience working with people with mental health and substance use disorders

    Minimum of a Master’s degree in Human Services, Social Work, Health or related field and two years of technical or professional experience working with Veterans in Human Services; or equivalent

    Experience interviewing and working with individuals in transition whose level of motivation may be limited or in need of enhancement

    Strong attributes of a role model for staff members and participants

    The Veterans Life Center is a 100-bed residential program designed to help at-risk male and female 21st-century military veterans ages 19 to mid-forties. We assist by providing the tools to strengthen their mental and physical health, improving life skills, and providing access / coordination to vocational training. Also guide these residents to career opportunities and community reintegration. Our programmatic goal is to avert the all-too-frequent crises such as chronic homelessness, incarceration, suicide, and premature death. Our comprehensive and systematic approach is designed to provide each of our residents the prospect of a constructive and personally rewarding role in civil society. The ultimate goal of the VLC is to help every resident achieve self-reliance.

    We have a rigorous assessment process, which enables us to identify the specific needs of each veteran. Once accepted, each veteran will work with a VLC case manager to develop their own reintegration program and be guided into one of our five tracks. Their length of stay varies dependent upon their needs.

    Residents live in double rooms in each of the three wings. The campus includes a computer lab, fitness center, a chapel, meditation garden, dining facility, library, two laundry rooms and dayrooms. We also provide limited medical assistance with our staff of on-site Nurses. Case Managers have their own private offices.
