Job Summary

Under the supervision of the Clinical Supervisor for the Citywide Supportive Housing Program Team, the Social Work Associate (SWA) works with San Franciscans living in Permanent Supportive Housing who struggle with issues of mental and physical health, substance use and housing retention to collaborate with community providers and link residents to appropriate outpatient services and provide housing stabilization to prevent homelessness.

The SWA will be assigned a caseload of residents from one of the DISH supportive housing sites (Camelot, Le Nain, Empress, Windsor, PBI, Star) and provide individual treatment including case management services, crisis intervention and 5150 assessments, substance use treatment, and psycho education, group treatment, community events; intake, initial and annual assessment and service plans and consultation plus other duties as assigned.

The SWA maintains client records according to California City and County requirements; and participates on an interdisciplinary treatment team. Attends weekly staff meetings and participates in all required training sessions. Plus, other duties as assigned. Demonstrates a commitment to anti-racism, social justice and the ability to engage and respect a diverse community of clients and peers.

The SWA will coordinate closely with property management staff from DISH, DPH Nursing and all support service partners, to develop treatment strategies; advocacy and coordination with other agencies, families, and clients; track client contacts in Epic and One Systems, complete initial and annual assessments, referrals, and linkage; coordinate closely with hospitals and clinic medical providers and the candidate will also perform other duties as assigned. Clients typically require routine assistance, individualized treatment approach and support from a community of providers.

Primary emphasis on treatment uses a client centered approach to prepare individuals with disabilities in attaining optimal function, including psychological, social, and vocational function in the context of their personal goals, abilities, perception of quality of life and maximize housing retention with the use of a combination of clinical case management and advocacy.

The job will involve the ability to lift 50 lbs, walking and climbing stairs and outreaching clients in their homes daily.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $29.79 - $38.74 (Hourly Rate).

The San Francisco General Hospital (SFGH) is dedicated to providing services that integrate medical, psychiatric, and addiction treatment. SFGH is the largest provider of acute psychiatric care and the only San Francisco provider of psychiatric emergency care.

Required Qualifications

* Possession of a bachelor's degree in social work from a program approved by the Council on Social Work Education, and one years of experience; or an equivalent combination of education and experience if a bachelor's degree is not possessed; and knowledge and abilities essential to the successful performance of the duties assigned to the position.
* The ability to do outreach, including SRO hotels, inpatient hospitals and scattered site housing locations.
* Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology

Preferred Qualifications

* Two years working with severely mentally ill adults, the homeless and dual diagnosed populations
* Two years Case Management experience
* Demonstrated ability to homelessness response system of SF
* Demonstrated understanding of UCSF values and Principles of Community.
