Pharmacy Coordinator

Position Summary :

This position is responsible for assisting pharmacist with managing the patients' disease states, including Diabetes, COPD, CHF and performing medication reconciliation. Follow up with the patients for 3 to 6 months, towards the patients' disease goals.

Essential Duties and Responsibilities include the following :

• Contact patients regarding their disease states
• Contact the providers office- as needed
• Collect blood sugars / blood pressure and pertinent information from the patient
• Communicate with the pharmacist regarding the patient's information
• Familiarize himself / herself with the main disease states : Diabetes, COPD, CHF
• Health education for the patients
• Blood sugar meter training
• Other duties as assigned

The pay range for this position at commencement of employment is expected to be between $21- $23 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and / or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
