*AIMIGO Home Health* *_Certified Occupational Therapist Assistant_* Supervised By: Occupational Therapist, Director of Rehab Supervisory Responsibilities: None Job Summary: Under the direction of the Occupational Therapist, provides Occupational Therapy services to patients by assisting in the treatment and implementation of programs, and related professional in-services. *DUTIES AND RESPONSIBILITIES *include the following essential duties: Provides Occupational Therapy services to patients and initiates referrals under the supervision of an Occupational Therapist in accordance with state regulations. Assists the Occupational Therapist in the evaluation process as needed or directed. Identifies indications for and assists in evaluations at appropriate intervals under the direction of the Occupational Therapist. Assists the Occupational Therapist in identifying a problem list, short-term goals, long term goals and treatment plan. Conducts individual, supervised and group treatment as assigned by the Occupational Therapist and as outlined in the treatment plan. Monitors patient's response to intervention and consults with Occupational Therapist if modification is indicated. Adheres to ordered frequency and OT Plan of Care/Treatment and informs OT of changes in need or condition that indicates a modification in the POC. Provides/recommends/fabricates adaptive devices or other equipment; trains patient and caregiver in the use or application of orthotics and prosthetics. Participates in patient, family and staff education. Confers formally and informally with other team members in coordinating the total rehabilitation program of the patient. Assumes responsibility for ongoing continuing education and professional development. Provides treatment in accordance with established standards of practice, department procedures and productivity standards. Records all services provided according to agency policies. Reports to work on time and coordinates schedule to assure ordered services are provided Maintains safe and clean work area and adheres to agency safety standards Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional. Maintains positive relationship and rapport with coworkers, patients and family members. Assists with and participates in Quality Assurance Programs as required Adheres to established confidentiality standards. Projects a positive and professional image at all times. Performs other duties as assigned. *_EDUCATION and/or EXPERIENCE:_* Associates Degree in Occupational Therapy from an accredited program Successful completion of the National Certification for Occupational Therapy Assistant Two years of occupational therapy experience required. Home health experience preferred *ABILITIES, SKILLS AND KNOWLEDGE:* Ability to read and interpret physicians orders and notes from other caregivers as well as respond to them verbally and in writing. Ability to effectively present information to clients and employees and community. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to calculate figures/amounts such as area, circumference, and volume and gauge distance. Ability to deal with several abstract and concrete variables. Ability to improvise in a home setting. Ability to use independent judgment to manage and impart confidential information. Skill to attend to multiple tasks at once which are time sensitive. Knowledge of principles and scope of occupational therapy. Basic knowledge of computer as required. Ability to establish and maintain effective working relationships with fellow employees, the public, and supervisors. *SPECIAL REQUIREMENTS:* Active Colorado Occupational Therapist Assistant license, and Current CPR certification; Current TB screening upon hire and annually thereafter; Reliable transportation to all assignments; and when transporting patients - possession of a valid State Driver's License and state required car insurance. *PHYSICAL REQUIREMENTS: * Ability to regularly sit, stand, stoop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with hands and arms and talk and hear. Ability to frequently use hands to manipulate tools or controls, handle, or feel. Ability to occasionally climb and use sense of smell. Ability to frequently lift and/or move up to 50 pounds and occasionally lift and/or move 100 pounds, to move objects, equipment or patients. Ability to perform the anticipated level of physical assist required to provide safe client care. If the anticipated level of assist is greater than that which the employee can safely provide, the supervisor is to be notified immediately. Ability to use close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. *WORK ENVIRONMENT: * The employee is frequently exposed to outside weather conditions. The work environment is highly variable, as care is provided in private homes. Lighting, acoustics, air quality, temperature and environmental hazards are not predictable. The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals. The noise level in the office work environment is usually moderate.
