Job Summary

Signature Point is seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will be responsible for assessing, planning, and implementing individualized treatment programs for patients across various settings, including inpatient, outpatient, and home health care. This role requires a strong foundation in physiotherapy principles, anatomy, and medical terminology, with a focus on providing exceptional patient care.

Responsibilities

• Conduct thorough evaluations of patients to determine their physical therapy needs.
• Develop and implement comprehensive care plans tailored to individual patient goals.
• Utilize various therapeutic techniques including trigger point therapy and orthopedic rehabilitation.
• Collaborate with interdisciplinary teams to ensure coordinated patient care.
• Provide education and support to patients and their families regarding treatment plans and home exercises.
• Document patient progress using EMR systems in compliance with healthcare regulations.
• Participate in discharge planning to facilitate smooth transitions for patients returning home or moving to other care settings.
• Maintain up-to-date knowledge of best practices in physiotherapy and engage in continuous professional development.
