Job Summary :

The Physical Therapist provides physical therapy services to patients in their homes within Hood River OR and surrounding areas. This role involves patient evaluation treatment planning and implementation of therapeutic interventions to improve patients physical function and overall well-being. The therapist is expected to manage a caseload maintain productivity standards and collaborate with other healthcare professionals.

Location : Hood River Oregon United States

Responsibilities :

• Conduct patient evaluations to assess physical condition functional limitations and rehabilitation needs.
• Develop and implement individualized treatment plans based on patient assessments and goals.
• Provide therapeutic interventions including exercise programs manual therapy and functional training.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and caregivers on proper techniques and home exercise programs.
• Document patient care services accurately and timely in the electronic medical record (EPIC).
• Communicate with physicians and other healthcare professionals regarding patient progress and treatment recommendations.
• Manage a caseload of patients ensuring efficient scheduling and productivity.
• Maintain compliance with all applicable regulations and standards of practice.
