Reporting to the Supervisor Ancillary Services, the Lead Physical Therapist will oversee daily clinical operations and deliver exceptional physical therapy across an advanced home health practice. Responsibilities include performing comprehensive evaluations, creating individualized evidence-based treatment plans, providing manual therapy, offers therapeutic exercise and neuromuscular re-education, supervises and mentors physical therapy staff, coordinates interdisciplinary care with physician and nursing staff, leads daily team huddles, ensures accurate and timely EMR documentation, maintains compliance federal and stage regulations, and leads quality improvement initiatives.

What's Needed to Succeed :

• Licensed as a Physical Therapist in the state of New York
• Proven leadership experience highly desirable
• Prior working experience in a home health setting
