You will...

Provide one-on-one skilled nursing care and treatment to patients in their homes.

Assist registered nurses in assessing patients' conditions and contributing to care planning.

Demonstrate expertise in home health and discipline-specific patient care skills.

Perform comprehensive patient assessments and collaborate with multidisciplinary care teams.

Monitor vital signs, administer treatments, and educate patients and families on medical conditions.

Manage patient episodes according to protocol.

Complete documentation accurately and timely.

You have...

Current and unencumbered LPN license

1+ year(s) of Certified Home Health experience as an LPN

Current CPR certification

Comfortable with technical tools and EMR for visit documentation

Reliable transportation, driver's license, and car insurance

Proof of Immunization / Exemption for Flu and COVID-19

TB Test or Chest X-Ray
