Position Summary

The Registered Nurse (RN) Care Manager / Clinical Diabetes Educator plays a vital role in providing comprehensive diabetes education and care for patients with Type 1, Type 2, gestational diabetes, and those at high risk. Education is delivered via phone, video teleconferencing, and in-person sessions, including village-based outreach. This role supports both direct patient care and community health promotion activities throughout the YK Delta Region. The RN Care Manager collaborates with multidisciplinary teams to uphold diabetes care standards and helps maintain the Diabetes Registry and grant compliance.

Key Responsibilities

Deliver individual and group diabetes education to patients in-person and virtually

Collaborate with Registered Dietitians, Pharmacists, and village clinic staff for coordinated care

Order lab tests and ensure results are managed per hospital policy

Document all patient interactions in the electronic health record (EHR)

Participate in and support regional diabetes outreach efforts, including support groups, media campaigns, and special events

Travel to remote villages multiple times per month to provide education and health promotion

Contribute to departmental planning and quality improvement initiatives

Uphold organizational values and support tribal partnerships in health efforts

Minimum Qualifications

Education & Certification :

Associate's Degree in Nursing (minimum required)

Bachelor's or Master's Degree in Nursing (preferred)

Current, valid Registered Nurse (RN) license in the State of Alaska (required)

Must obtain CDCES (Certified Diabetes Care and Education Specialist) certification within 2 years of hire

Basic Life Support (BLS) certification preferred

Experience :

New graduates with active RN license may be considered

Two (2) years of experience in a clinical healthcare setting preferred

Prior experience in diabetes education and care coordination strongly preferred

Proven ability to work in cross-functional teams and with diverse populations

Additional Requirements :

Proficiency with computers and EHR systems

Ability to operate basic medical equipment (e.g., glucometers, scales)

Must be able to travel by small aircraft and work in sub-arctic environments

Strong communication and cross-cultural skills are essential

Bilingual ability (Yup'ik / English) preferred
