Job Overview
As a Transition of Care Clinical Pharmacist at Linea, you’ll lead medication optimization and safety during the critical 90-day recovery window following hospital discharge. You’ll review complex medication regimens, identify opportunities to improve therapy, and collaborate closely with prescribers to advance evidence-based, guideline-directed care for patients with heart failure, CKD, ESRD, and diabetes.
Beyond direct patient and provider support, this role offers the opportunity to shape Linea’s evolving care model—helping design the clinical workflows, tools, and decision-support processes that power our scalable, technology-enabled care platform.
Key Responsibilities
Comprehensive Medication Review & Optimization
- Conduct detailed chart reviews, medication reconciliations, and therapy assessments for patients transitioning from hospital to home.
- Identify drug-related problems, adherence barriers, and therapeutic gaps, particularly for patients with heart failure and renal comorbidities.
- Provide actionable recommendations to optimize treatment in alignment with national guidelines
Clinical Collaboration & Decision Support
Communicate clearly with prescribers (cardiology, nephrology, primary care) to recommend therapy changes with supporting rationale and literature references.Collaborate with nurse practitioners, care navigators, and physicians to implement treatment adjustments and manage high-risk medications.Contribute to deprescribing and medication simplification initiatives to reduce polypharmacy and enhance safety.Specialized Disease State Management
Apply deep expertise in heart failure pharmacotherapy (HFrEF, HFpEF, HFmrEF), including dose titration and monitoring.Manage cardiorenal-metabolic overlap—balancing diuretics, nephroprotective agents, and glycemic control.Support management of anemia, electrolyte disorders, and fluid balance in CKD and ESRD.Product and Process Development
Partner with clinical operations and engineering teams to design and refine medication workflows, dashboards, and protocols.Define clinical rules, dosing algorithms, and titration sequences used across Linea’s digital tools.Analyze process metrics (e.g., time-to-reconciliation, GDMT optimization rates) and identify opportunities for continuous improvement.Education & Quality Improvement
Develop internal medication management resources, playbooks, and training materials for the clinical team.Create patient-friendly medication education content to support adherence and understanding.Contribute to clinical quality initiatives, data reviews, and outcomes reporting tied to pharmacist interventions.Qualifications
Required :
Doctor of Pharmacy (PharmD) from an ACPE-accredited institutionActive pharmacist license in good standing (multi-state licensure a plus)2–3 years of clinical pharmacy experience with expertise in cardiology, nephrology, or internal medicineStrong knowledge of GDMT protocols, renal dosing, and cardiometabolic pharmacotherapyExperience collaborating within multidisciplinary teams and communicating with prescribersProficiency with EHRs and comfort using digital documentation and communication toolsPreferred :
PGY1 / PGY2 residency in ambulatory care, internal medicine, or cardiologyBoard certification (BCPS, BCACP, or BCCP)Experience in transitions of care, MTM, or chronic disease management programsFamiliarity with telehealth, remote monitoring, or care coordination modelsExperience contributing to clinical workflow or software developmentCore Competencies
