Home Health Registered Nurse (RN) - Phoenix

Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.\n\nWe are seeking an experienced and dedicated Registered Nurse (RN) to join our team on a contract basis for a client in Phoenix. If you are passionate about patient care and have a strong background in home health, we would love to hear from you! (OASIS experience is required)\n\nKey Responsibilities : \n

• Provide comprehensive nursing care to patients in their homes.\n
• Perform initial and ongoing patient assessments, including vital signs, medication management, and wound care.\n
• Develop and implement individualized care plans based on patient needs.\n
• Educate patients and their families on disease management, treatment plans, and preventive care.\n
• Collaborate with physicians, therapists, and other healthcare professionals to ensure a cohesive care plan.\n
• Document all patient care activities accurately and promptly in electronic medical records (EMR).\n
• Monitor and report changes in patient condition to the supervising physician.\n
• Ensure compliance with all state and federal regulations related to home health care.\n\nQualifications : \n
• Active RN license\n
• Minimum of 3 months of home health experience using OASIS\n
• HCHB experience is a big plus\n
• Strong clinical skills, including IV therapy, wound care, and medication management.\n
• Proficient in using electronic medical records (EMR) systems.\n
• Excellent communication and interpersonal skills.\n
• Ability to work independently and manage time effectively.\n
