Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Part -Time Registered Nurse (RN) in Nashua, NH. The Registered Nurse delivers care to individuals with disabilities in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing. Responsibilities
- Provide treatment and health care procedures ordered by a physician and required by the
standards of professional practice or law
Ensure individuals attend medical appointments per their Individual Support Plan (ISP)Advise supervisor of all concerns and issues related to the provision of nursing servicesCreate and distribute medical / health reports detailing general health, health care goals, andphysician recommendations
Conduct programmatic trainings including medication administration and delegationDevelop and update Individual Support Plans (ISP) / Person Centered Plans (PCP) with aninterdisciplinary team to include any concerns, observations, and behavioral developments
Train employees on the medical needs of assigned individualsMonitor health and medical conditions of individuals and refer for medical services as neededMonitor the use of medications through medication administration logsEnsure health service supplies are available and replenished as neededEnsure program documentation and billable records are completed accurately and timelyBuild a support network for individuals by developing and maintaining relationships withcommunity members, businesses, and families
Participate with individuals in community, social, and recreational activitiesParticipate in circles of support for personal futures planningFollow strict OSHA procedures regarding blood borne pathogensAdditional tasks and responsibilities may be assignedEmployee must cooperate with the license and department staff in any inspection, inquiry, orinvestigation

Minimum Requirements
Bachelor’s degree or equivalent experience in nursing fieldMinimum of one year of experience providing nursing services to individuals with disabilitiesValid R.N. licenseValid driver’s license with a satisfactory driving recordComplete all state and agency required training per state guidelinesAbility to calculate figures related to medication dosagesUnderstanding and commitment to community-based support for persons with disabilitiesProficient with Microsoft OfficeAbility to effectively communicate with program participants, third-party agencies, staff, andmembers of the community
Working Conditions
