Responsibilities :
Accept and treat patients within your designated geographic areaEvaluate patients needs for physical therapy in the home setting and develop individualized treatment plansDevelop a plan of care and treatment goals in consultation with the physician patient and familyPlan prepare and implement customized therapy programs to restore function and promote independenceInstruct family members PTAs Home Health Aides and caregivers on appropriate therapy techniquesProvide timely and accurate documentation for each visitCommunicate effectively with patients families and interdisciplinary team members especially around discharge planningDeliver care in compliance with agency policies and professional practice standardsQualifications :
Physical Therapist Program GraduateLicensed Physical Therapist in the state of New York or eligible.Mandatory Certifications : First Aid AED & CPR Certifications.Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.Ability to travel to patients homes within the assigned geographic territoryAdditional Information :
