About HealthBridge
HealthBridge is currently a virtual-only clinic that serves complex, recently discharged patients
through Remote Patient Monitoring, Chronic Care Management, and Transitional Care. We are
opening our first in-person clinic in Richmond. The goal is to build a modern primary care model
that starts with post-hospitalization care, expands into traditional primary care, then layers a
hybrid program focused on longevity and lifestyle.
The Opportunity
This is a founding role. You will help design, launch, and scale our first in-person clinic. You will
create the playbook, assist in hiring the team, help stand up operations, and start caring for
patients. Once additional providers are onboarded, you’ll shift panels and drive the development
of new clinical and operational functions.
What you will do
Build the clinic
- Revamp current clinical workflows for post-discharge, TCM, RPM, and CCM.
- Stand up SOPs, policies, and quality protocols for in-office visits.
- Specify equipment and supplies.
- Integrate our existing virtual-only care model into the new physical clinic setting.
Build the team
Play a role in building the team, and mentor clinicians.Contribute to shaping staffing models, coverage plans, and escalation pathways.Collaborate on creating onboarding and ongoing training programs.Deliver care and outcomes
Start a patient panel, then transition to new clinicians as they come on.Ensure fast post-discharge access and tight follow-up.Use data to mitigate readmissions and improve patient experience.Lead and scale
Own clinical quality metrics and help shape the operating cadence.Partner with operations and product to improve workflows and tools.Help design the roadmap for traditional primary care and future longevity services.What success looks like in year one
Clinic launched with reliable 7- and 14-day post-discharge access.SOP library, training tracks, and QA audits in place.Readmission rate trending below target, strong patient satisfaction, and scalable staffing ratios.Second clinician onboarded and carrying a transitioned panel.Qualifications
MD or DO with board certification in Internal Medicine or Family Medicine.Active Virginia license or ability to obtain quickly.3-7+ years in primary care or hospitalist medicine. Start-up or clinic launch experience is a strong plus.Comfortable with telehealth, RPM, and CCM workflows.Systems thinker who writes clear SOPs and manages change.Collaborative leader who enjoys hiring, training, and coaching.Nice to have : Experience with geriatrics, cardiometabolic disease, care transitions, palliative care, longevity medicine, or value-based care.Compensation and benefits
