Your Role

As the RNAC, you’ll oversee and coordinate the resident assessment process (MDS), ensuring documentation accuracy, compliance, and individualized care planning.

Key Responsibilities :

Coordinate the development and completion of MDS assessments per federal and state regulations

Update and maintain MDS and care plan policies and procedures

Participate in facility surveys and ensure regulatory compliance

Assess residents to determine individualized care needs

Collaborate with the DON and interdisciplinary teams to support care planning

Provide training and oversight to ensure accurate, compliant documentation

What You’ll Bring

Active RN license in Pennsylvania (required)

Minimum 2 years of RNAC experience in a long-term care setting (required)

Proficiency in PDPM, CMI, MDS, and managed care processes

Strong understanding of state and federal regulations

Excellent communication, organization, and leadership skills
