Travel RN Job Details

Position Type : Travel Registered Nurse – Home Health

Location : Portage, Michigan 49002 (serving North / East Kalamazoo County; occasional coverage in Western Calhoun / Barry County)

Schedule : Monday-Friday, 8 : 00am – 5 : 00pm

Shift Duration : 8-hour day shifts

Assignment Length : 13 weeks (Start : 10 / 13 / 2025, End : 1 / 11 / 2026)

Hours per Week : 40

Estimated Weekly Salary : $1920–$1970

Registered Nurse Qualifications

Active Registered Nurse (RN) license in Michigan

Minimum 1 year of recent Home Health RN experience

Proficiency with HCHB (Homecare Homebase) and OASIS documentation systems

Strong clinical assessment and critical thinking skills

Excellent communication and organizational abilities

Home Health RN Responsibilities

Deliver skilled nursing care to patients in their homes throughout Portage and Kalamazoo County

Conduct comprehensive patient assessments and create individualized care plans

Document patient care using HCHB and OASIS systems

Collaborate with interdisciplinary healthcare team members for optimal patient outcomes

Educate patients and families on disease management, medication administration, and preventive care

Travel within North / East Kalamazoo County and occasionally support Western Calhoun / Barry County
