Job Summary We are seeking a dedicated and skilled Occupational Therapist to join a Home Health team providing high-quality care to patients in the Sacramento area. This role focuses on delivering patient-centered therapy services, managing documentation within required timelines, and maintaining clear communication with supervisors and office staff. Key Responsibilities Prioritize and complete Start of Care (SOC) visits. Supervise Occupational Therapy Assistants as needed. Submit next-day reporting for all Start of Care visits. Complete clinical notes and documentation within 48 hours of each visit. Maintain ongoing communication with supervisors, intake coordinators, and office personnel. Remain responsive and available during regular business hours. Provide therapeutic services according to the patient's plan of care. Attend mandatory training (3 full days : 1 in-house, 2 field-based). Travel to patient homes within the assigned territory. Requirements Active California Occupational Therapist (OT) License . Minimum 1 year of Home Health experience . Ability to manage productivity expectations (30 points per week). Reliable transportation for travel between patient visits. Documentation & Tools EMR System : Kinnser Equipment Provided : iPad and RingCentral access Mileage Reimbursement : $0.655 per mile (first to last patient) Parking and tolls : Reimbursed Productivity Structure 30 points per week required Visit Point Applications will be accepted only from candidates who meet the minimum qualifications and hold the necessary certifications.
