Description :

This is a field Hospice position requiring travel into patient homes and long-term care facilities.

The RN will serve assisted living and skilled nursing facilities as well as personal homes.

Areas served may be subject to change.

Expectations include availability during operating hours Monday through Friday.

There is opportunity to pick up night and weekend shifts if desired.

A minimum of 1-2 years of home visit hospice experience is required.

The RN will work with end-of-life and hospice-eligible patients.

This is a field position requiring the traveler to commute and provide care within home settings.

The interdisciplinary team holds biweekly case conferences to discuss patient load.

RN will document IDG notes showing continued eligibility for hospice services.

Mileage reimbursement will be provided at the current IRS rate.

Average caseload will be around 14-15 patients, with an ideal goal of 5-6 visits per day.

The traveler will have autonomy in managing their own schedules.

Drive time will vary based on the served region.

Strong listening and communication skills are required, alongside assessment capabilities of patient and family needs.

Experience with catheter insertion and care, feeding support including G-tubes, and medication management is needed.

The RN must be able to provide psychosocial support and bereavement support.

Ability to teach families and patients about end-of-life issues and expected declines throughout the process is essential.

Support from peers, nurse educators, clinical supervisors, and an interdisciplinary team will be available.

Technology will include EMR systems for documentation and a provided tablet and laptop for home charting.

Orientation and training will be provided by experienced preceptors.

Call requirements and holiday expectations will be discussed during the final interview.
