Qualifications :
- Education : Bachelor’s Degree or higher by an accrediting agency recognized by the US Dept. of Education in HealthCare related field is required.
- Experience : Prefer 2-5 years’ experience in health care related field with at least 1 year supervisory experience. Minimum of 1 years’ experience working with the frail elderly population.
Essential Functions :
Assure success of the participant by holding accountable of IDT members.Coordinates all IDT Meetings including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and all related follow up. Directs and oversees the IDT members. Hires, trains, evaluates and disciplines IDT Members in coordination with the Center Administrator and applicable department Managers.Coordinate staff development and training and assists with other human resource issues in conjunction with Human Resources Department. Responsible for the implementation, coordination, and quality improvement processes as it relates to the Interdisciplinary team.The Team Coordinator is responsible for the development, implementation and execution of participant care. Facilitates activities related to new participants in coordination with the IDT.Performs routine care plan audits and all related follow up with disciplines’ interventions and evaluations in participants’ care plans. Ensures that team members understand the importance of their and others’ roles on the team and remain open to the cooperative dynamic of the team.Participates in the continued development of the team to improve the team decision-making process. Provides support to team to troubleshoot, advice, teach, coordinate and deliver care and service to participants.Ensures the provision of quality service within expected standards of care. Coordinates orientation and in-service programs for staff to meet regulatory requirements and support performance improvement.Functions as a member of leadership team with the Carolina SeniorCare and promotes the mission and vision of leadership in a positive and productive manner.Other duties as assigned.Salary : Based upon skills and qualifications
