Care Manager – EverCare Mobile Health

Location : Eastern Idaho - Idaho Falls

Schedule : Full Time

Compensation : $20 - $28 per hour

All applicants must have one of the following qualifications :

• A  Bachelor's degree  in Psychology, Social Work, Healthcare, or a related helping field, OR
• A  current Certified Peer Support Specialist (CPSS) certification .

L1 :  Care Partner is new to the industry or new to Community Health Integration (CHI).

Wage range :  $20 - $22

L2 :  Individuals have been with the organization for one year. Or has a CPSS Certification.

Wage range :    $22 - $24

L3 :  Individual must have a Bachelor degree.

Wage range :  $24 - $26

L4 :   Care Manager -  Must have a BA or BS in Social Work or Psychology

Wage range :  $26 - $28
