Job Description\n

American Traveler is seeking an experienced RN for a Hospice and Home Health position in Vermont, requiring a VT or compact RN license and at least 2 years of relevant experience.

\n \n Responsibilities\n

  • Provide nursing care in home health and hospice settings
  • Serve adult and geriatric patient populations in their homes
  • Work schedule is Monday through Friday from 8am to 4pm with some weekend and on-call requirements
  • Role includes case management and coordination with interdisciplinary teams
  • Responsible for implementing care plans, performing assessments, and providing patient and caregiver education
  • Provide comprehensive assessments and develop individualized care plans for patients and caregivers
  • Act as case manager, managing caseloads and visit schedules according to productivity standards
  • Complete all required documentation including agency, state, and federal forms accurately and in a timely manner
  • Educate patients and caregivers on medication side effects, treatments, discharge instructions, and services available
  • Participate in monthly peer review audits and interdisciplinary team meetings
  • Adhere to infection control protocols and maintain agency standards of care throughout all processes
  • Work collaboratively with LPNs and other care team members to ensure quality outcomes

  • Active VT or compact RN license required
  • Minimum of 2 years’ RN experience in home health or hospice
  • Competency in OASIS documentation and completion in line with agency and regulatory requirements
  • Ability to perform high-tech skills such as IV use and maintenance, wound VAC, and complex wound care
  • Experience with interdisciplinary collaboration and care coordination
