Job Responsibilities :
Evaluate residents referred for speech, language, cognitive-communication, voice, and swallowing disorders.
Develop and implement individualized treatment plans based on each patient’s needs, goals, and tolerance.
Provide direct therapy to improve communication skills, cognitive function, and safe swallowing abilities.
Set realistic, achievable goals and track progress throughout the plan of care.
Accurately and promptly complete documentation, including evaluations, progress notes, re-certifications, and discharge summaries.
Educate patients, families, and staff on therapy techniques, communication strategies, and safe swallowing practices.
Collaborate with all interdisciplinary team members to ensure patient-centered, coordinated care.
Maintain compliance with HIPAA, confidentiality standards, and Federal Residents’ Rights.
Perform additional duties as assigned by the Director of Rehabilitation.
Requirements :
Graduation from an accredited Speech-Language Pathology program (Master’s required).
Valid state SLP license and registration (CCC-SLP preferred but CFYs welcome).
