What We Are Looking For :

  • Accountability for Outcomes : You will manage a diverse panel of up to 450 patients, influencing their health outcomes through trust and behavioral support.
  • Health Coaching : Engage with patients as a partner in their health journey, setting and following up on personalized goals for improved well-being.
  • Simplified Actions : Help patients navigate their health journeys by prioritizing actionable steps.
  • Team Leadership : Lead a dedicated care team to deliver exceptional service and cultivate a positive workplace culture.

Your Essential Responsibilities :

  • Provide comprehensive care for diverse acute and chronic conditions affecting older adults.
  • Oversee patient care and collaborate with specialists to ensure the best outcomes, maintaining involvement through all stages of care.
  • Engage with hospitalists and transitional care teams to manage patients effectively across all settings.
  • Play an active role in center management, recruitment, and continuous improvement of services.

    • Required Qualifications :

  • Medical Doctor (MD) or Doctor of Osteopathy (DO) with specialization in Internal Medicine, Family Medicine, Geriatrics, or a relevant field.
  • Active state medical license and board eligibility with a commitment to maintaining certification.
  • Strong communication skills and a patient-centered approach to care.
  • Passion for serving seniors and enhancing their quality of life.
