Avani Healthcare West is seeking a local contract Speech Language Pathologist for a local contract job in Flagstaff, Arizona.

Role : Speech Language Pathologist

Location : Flagstaff, AZ

Shift :  08 : 00 AM – 05 : 00 PM

Duration : 28 weeks

Schedule : 40 hours per week / 5 days

Pay Range :   Hourly rate - $55 - $60 per hour

Job Description :

  • Screens and evaluates patients / residents and develops appropriate care plan.
  • Under a physician's direction, provides direct patient / resident treatment based on established plan of care.
  • Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
  • Administers, scores, and interprets specialized hearing and speech tests.
  • Instructs residents / patients, families, and caregivers to monitor speech and provide ways to practice new skills.
  • Completes required forms and clinical records in accordance with company policy and state and / or federal regulations.
  • Documents in patient records in an accurate and timely manner.
  • Devises special tools and treatments for the specific needs of patients.
  • Provides comprehensive and compassionate therapy services in a sub-acute / skilled nursing facility setting.
  • Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.
  • Maintains resident / patient privacy pursuant to HIPAA.
  • Ensures punctuality and regular attendance for assigned shifts.

    • Knowledge / Skills / Abilities

  • Current knowledge of effective speech therapy techniques.
  • Familiarity with clinical needs of geriatric population.
  • Strong clinical capabilities.

    • Requirements :

  • Active license in Speech Language Pathology in the Stat of Arizona.
  • Bachelor of Science degree in Speech Language Pathology required.

    • Certifications :

  • BLS (American Heart Association ONLY)
  • Level 1 Fingerprint Clearance Card
  • OIG / SAM name clearance check
  • TB Skin Test
  • COVID vaccination or declination of offered vaccine

    • Role : Speech Language Pathologist

    Location : Flagstaff, AZ

    Requirements :

  • Active license in Speech Language Pathology in the Stat of Arizona.

  • OIG / SAM name clearance check
  • TB Skin Test
  • COVID vaccination or declination of offered vaccine
