Position Summary :
The Director, Clinical Pharmacy provides strategic and operational leadership for pharmacy medical management initiatives that improve healthcare quality, cost-effectiveness, and outcomes. This role ensures pharmacy programs align with the organization's mission to optimize medication use and support value-based care. The Director collaborates with internal and external stakeholders—including physicians, nurses, pharmacists, and leadership—to drive clinical, financial, and operational performance through evidence-based medication management and innovative pharmacy models.
Key Responsibilities :
Clinical Strategy & Program Leadership
- Develop and execute clinical pharmacy strategy aligned with value-based care and population health.
- Integrate pharmacy programs into workflows across primary care, specialty care, care management, and utilization management.
- Lead initiatives such as chronic disease management, medication therapy management (MTM), transitions of care, site of care, deprescription, and comprehensive medication reviews.
- Establish evidence-based protocols, formulary strategies, and quality improvement programs.
- Collaborate with providers to optimize prescribing and reduce medication-related adverse events.
- Provide input on pharmacy payment integrity initiatives
Operations & Compliance
Manage clinical pharmacy staff : hiring, training, performance, and development.Ensure compliance with state / federal regulations, accreditation standards, and payer requirements.Oversee departmental budgets, performance metrics, and operational reporting.Monitor quality of care and regulatory compliance.Data, Analytics & Performance
Use EMR, pharmacy analytics, and population health tools to guide decisions and measure impact.Support value-based contracts and quality metrics (e.g., HEDIS, STAR, CMS).Collaborate with IT and analytics teams to enhance clinical decision support and medication data integration.Identify high-risk pharmacy claims and trends through data systems.Stakeholder Engagement & External Relations
Serve as liaison between pharmacy services, care teams, and administrative departments.Represent pharmacy leadership on committees focused on quality, formulary, and patient safety.Build relationships with external partners : payers, PBMs, suppliers, and academic institutions.Qualifications :
Education & Licensure
BS in Pharmacy and PharmD required.Active pharmacist license in good standing (state-specific).Postgraduate training preferred.Board certification (e.g., BCPS, BCACP, BCGP) strongly preferred.Experience
8–10 years of progressive pharmacy experience, including 3–5 years in clinical leadership.Proven success in developing pharmacy programs within health systems, ACOs, or integrated care organizations.Experience with population health, value-based care, and pharmacy analytics.Skills & Competencies
