Pay : $18 / hour

Work Schedule : Monday - Friday 8am - 4 : 30pm; temporary position lasting approximately 3-6 months

This position is eligible for Paid Sick and Safe Time (PSST) under the Healthy Families and Workplaces Act. All employees may also make pre-tax or post-tax (Roth) contributions to our retirement plan – must be 18 years of age to participate.

JOB SUMMARY :

The Case Manager I, Supported Employment will develop and implement a program for all assigned clients in the Supported Employment program. Providing job coaching and ongoing support is necessary to maximize their independent functioning in a work and community environment. To provide intensive support services to develop the abilities necessary to maximize their independent functioning.

ESSENTIAL FUNCTIONS :

Program Case Management :

  • The Case Manager will provide direction, supervision, and case management for all assigned clients. The incumbent will complete an initial rehabilitation plan and assessment, develop, and implement an individual program plan, and document progress for the participant. The Case Manager will function as part of the Interdisciplinary Team (IDT) to plan and develop the individual rehabilitation program, plan ancillary activities for clients to include special staffing and initiate appropriate referrals for special services when necessary. It is critical that the incumbent have the ability to encourage independence by assisting clients in problem solving their personal and work-related problems by using accepted and innovative rehabilitation techniques and maintaining a proactive environment. In addition, the incumbent will teach proper work habits, interaction skills, and self-help skills through role modeling and programmatic intervention. The Case Manager will facilitate training in the production methods, increase productivity and assist in remedial training when necessary. It may also be necessary for the incumbent to assist with medication administration, assist with assigned participant transportation by completing all participant pickups and drops offs and assist in any needed personal care of the program participant.

Placement Coordination :

  • The Case Manager will provide ongoing support to the employer and client. The incumbent will provide job coaching, systematic instruction, and facilitate natural support structures while educating potential employers on such topics as disability awareness, Americans with Disabilities Act (ADA), and how to supervise an intellectual and developmentally disabled person.
  • Participant / Program Tracking and Reporting :

  • The Case Manager must complete all necessary documentation, e.g., incident reports, program plans, job placement paperwork, annual assessments, Individual Service Plans (ISP), data tracking, quarterly and final reports. It is critical that the incumbent maintain complete and accurate records and statistical data as required for all programs.
  • Relationship Management :

  • The Case Manager will communicate and coordinate with other involved agencies and support personnel, e.g., Residential staff, case management agency, host home providers, parents and guardians, The Department of Vocational Rehabilitation, and schools. The incumbent will keep the Program Manager informed of progress, considerations, and pertinent information; communicate significant problems to the Supervisor and deal effectively, and calmly with non-routine situations following accepted guidelines. In addition, the Case Manager will participate in team meetings and in-service training sessions. The Case Manager will follow all guidelines and perform other duties as required by the Program Manager.
  • Research and Industry Involvement :

  • The Case Manager is required to research, keep updated by reading trade journals as needed to be aware of trends, theories or initiatives within job development and placement industry. The incumbent will seek out seminars and symposiums for ideas and stay current with trends in new software, tools, and resources available. The Case Manager must also maintain a working and current knowledge of Rules and Regulations and CARF, and a good working knowledge of Goodwill’s policies and procedures.
  • QUALIFICATIONS :

    Education :

  • A Bachelors Degree in Vocational Rehabilitation, Psychology, Sociology, Counseling and Guidance, or related degree is preferred.
  • Experience :

  • Applicable fields or equivalent combination of training and experience in a social service, counseling, rehabilitation setting, job coaching, providing direct services to persons with physical, emotional, mental and / or developmental disabilities is preferred
  • Other :

  • It is preferred that the incumbent have excellent written and verbal communication skills and demonstrate the ability to interact with various types of internal and external customers. It is also preferred that the incumbent have the ability to relate effectively to disabled persons, their parents and guardians and be empathetic towards their needs.
  • The incumbent must have the ability to work independently, cooperatively and to exercise effective planning.
  • The incumbent must have a high degree of maturity, good judgment, responsibility, and initiative, must possess effective interpersonal skills including the ability to make decisions, interact with the public, work cooperatively as well as independently and organize work.
  • The incumbent must be able to write clear, concise, and accurate reports and plans, submit them in a timely fashion and possess excellent time management skills.
  • Additionally, the incumbent must have a working knowledge of community resources, have the ability to train, and be creative in motivating clients toward individual success.
  • The incumbent must be capable of dealing with conflict effectively between employers and the client as well as dealing with other funding agencies, be neat and personable in appearance.
  • The incumbent must also conduct self in a professional manner and maintain a professional appearance that enhances the firm’s reputation.
  • Must be flexible, available to work any hours as needed to complete the job. Some evenings and weekends may be included.
  • The incumbent will be required and must have the ability to drive for company business
  • Per auto vehicle insurance carrier requirements : For applicants / employees to qualify for inclusion on the Goodwill driver insurance schedule, for purpose of driving Goodwill fleet vehicles, or for purpose of receiving company mileage reimbursement they must be at least :
  • 19 years of age (not engaged in passenger transportation),
  • 25 – 70 years of age for all passenger transportation services,
  • 21 years of age for CDL.
  • Applicants / employee will be required to undergo a Motor Vehicle Record (MVR) check and background check; applicants / employees must disclose all moving traffic violations or vehicle crashes (within the last five (5) years)(Note : not all violations are a disqualification for employment and each case will be reviewed by the insurance carrier); must have valid Colorado State driver license; must also be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.

    Applicants / employees using their personal vehicle for company business (receiving company mileage reimbursement), must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition.
