As a Clinical Manager (RN), you’ll lead an interdisciplinary team that delivers life-changing care to patients across Suffolk County.

Your Impact :

As Clinical Manager, you’ll play a key leadership role in overseeing patient care services and empowering our dedicated field clinicians and support staff. You’ll ensure quality, compassionate, and compliant care is delivered across a patient census of approximately 200 individuals.

You will :

Lead and support an interdisciplinary care team, including field staff and independent contractors

Promote excellence in patient care through clinical oversight, mentorship, and performance evaluation.

Drive quality improvement initiatives in partnership with our QAPI and Education team.

Ensure integrity in documentation, OASIS reporting, and compliance standards.

Guide staff development and professional growth with a focus on compassionate, evidence-based care.

Participate in shaping agency-wide policies, procedures, and quality assurance initiatives.

What You Bring :

RN license in New York State (Required)

Bachelor’s degree in nursing (Preferred)

Experience : 2+ years in home care with a BSN, or 4+ years in home care with additional training in public health / management, minimum of 1 year of leadership experience in an RN role.

Strong interpersonal and leadership skills

Familiarity with home health care systems and regulations

Commitment to therapeutic communication and holistic, patient-focused care

Salary Range :

$120,000.00 - $150,000.00 per year
