    • Position Overview
    • Aeris Medical Group is seeking an experienced

    • Physician
    • to join our growing post-acute care team. In this role, you will lead and deliver comprehensive primary care to residents in SNF and ALF settings, focusing on prevention, chronic disease management, and care coordination. This position offers clinical autonomy, flexible scheduling, and the opportunity to practice medicine the way it was meant to be—personal, thoughtful, and impactful.
    • You'll work alongside a dedicated team of Nurse Practitioners, Clinical Coordinators, and Operations staff who handle the administrative load so you can focus on patient care.

    • Responsibilities
    • Perform patient assessments, routine visits, and follow-ups.
    • Diagnose, manage, and treat acute and chronic medical conditions.
    • Coordinate care with NPs, facility nursing staff, and families.
    • Order and interpret diagnostic tests, labs, and imaging.
    • Provide medical oversight, collaboration, and education to advanced practice providers.
    • Document visits accurately and contribute to quality and compliance initiatives.
    • Engage in care planning, advance care discussions, and transitions of care.
    • Qualifications
    • MD or DO with an active, unrestricted Minnesota medical license.
    • Board Certification in Internal Medicine or Family Medicine required.
    • 2+ years of experience in geriatrics, post-acute, or primary care strongly preferred.
    • Strong communication, leadership, and collaboration skills.
    • Compassionate, patient-centered approach with a commitment to quality and continuity.
