The RN Care Manager will coordinate concurrent utilization management activities hospital-wide and act as the primary link between the patient, healthcare team, organization, and payors during the patient's hospital stay. Responsibilities : Conduct patient needs assessments, identify resources for transitions, and secure necessary support. Collaborate with, educate, and lead interdisciplinary teams in daily care rounds, care conferences, and care planning. Communicate effectively in written, verbal, and electronic formats (email and hospital software applications). Coordinate with community organizations for durable medical equipment, insurance authorizations, and transportation. Monitor and manage inpatient hospital length of stay. Apply knowledge of hospital reimbursement methods and quality measures relevant to care management. Requirements : Experience : 3-5 years of clinical experience in medical / surgical nursing, home care, hospital case management, and / or utilization review. Must have utilization review and discharge planning experience. Licenses / Certifications : Active Registered Nurse license (Associate or Bachelor degree-prepared). Active BLS required. CCM or ACM preferred. Certification by a nationally recognized case management or utilization review organization preferred.
