We are seeking a skilled and compassionate Nurse Practitioner (NP) to join our Central / East Valley Mobile Service healthcare team. As a Nurse Practitioner, you will provide high-quality, patient-centered care to individuals in their homes and community settings. You will assess, diagnose, and treat a variety of medical conditions while working collaboratively with a multidisciplinary team to ensure the best outcomes for every patient.

Key Responsibilities

• Perform comprehensive physical examinations and health assessments
• Order, perform, and interpret diagnostic and laboratory tests
• Diagnose and manage both acute and chronic medical conditions
• Prescribe medications, therapies, and other appropriate treatments
• Educate and counsel patients on preventive health measures and lifestyle management
• Collaborate with physicians, nurses, and allied health professionals to develop and implement care plans
• Monitor patient progress and modify treatment as necessary
• Utilize telehealth technology to provide virtual consultations and follow-up visits
