Job Description :
The Social Worker is based at 1-2 Unity Point Health - Meriter primary care clinics and addresses psychosocial and coordination needs for primary care patients of UnityPoint Health - Meriter. This position coordinates care for patients across the lifespan who would benefit from high-touch collaboration, advocacy, and care coordination. Special areas of focus are patients with high risk medical needs (eg multiple chronic conditions, cognitive difficulties, independence concerns, suicidality, etc.), and those with complex social determinants of health needs or barriers (e.g. abuse and neglect, homelessness, financial insecurity, etc.). The Social Worker provides leadership and representation of the ambulatory social work perspective system-wide through providing staff education, acting as a social work content expert, and participating in appropriate system initiatives.

Required Qualifications :
Masters Level Degree in Social Work from an accredited School of Social WorkExperience in care coordination including comprehensive, evidence informed social work assessments, and developing plans of careExperience communicating directly with patients and families and facilitating plan of care conversationsExperience working with multicultural populations and patients with complex psychosocial needsCurrent social work licensure in the state of Wisconsin. Minimum licensure requirement : Certified Advance Practice Social Worker (CAPSW)Valid driver's license when driving any vehicle for work-related reasonsHighly effective verbal and written communication skillsAbility to develop and implement new programs and initiativesKnowledge of social work code of ethics and ability to effectively integrate ethics into programs, processes and patient careStrong computer and data analysis skillsAbility to provide leadership and effective constructive feedback while working collaboratively in an interdisciplinary teamExcellent problem-solving, organizational, interpersonal and time management skillsStrong understanding of available community resourcesPreferred Qualifications :
Preferred focus in healthcareMinimum of three years post MSW in ambulatory healthcare settings preferredExperience managing other projects (eg data management, outcomes evaluation, workflow development, and ongoing program growth)Experience having Advance Care Planning conversationsAdditional certifications or education in related areas (case management, dementia, motivational interviewing, advance care planning, etc.)Use of EPIC and CareLinkUse of Microsoft Office Suite programs (eg Word, Excel, Outlook, Power Point, etc.)Knowledge of healthcare systemsEEO / M / F / D / V
