Job Overview :

We are seeking an experienced Director of Case Management to lead and oversee the case management department in Rutherfordton, NC. This leadership role is responsible for care coordination, discharge planning, and utilization review, ensuring the highest quality of patient care while optimizing hospital resources. The ideal candidate will have strong clinical expertise, leadership skills, and knowledge of case management best practices.

Key Responsibilities :

Oversee case management operations, ensuring efficient care coordination, discharge planning, and utilization management.

Develop and implement strategies to enhance patient care, reduce hospital readmissions, and optimize resource utilization.

Ensure compliance with state, federal, and accreditation standards, including Medicare and Medicaid regulations.

Lead and mentor case management staff, promoting professional growth and high performance.

Collaborate with physicians, nursing teams, and social workers to support patient-centered care.

Monitor case management metrics and implement process improvements based on data-driven insights.

Requirements :

Associates Degree in Nursing required.

BLS Certification required.

Active RN license to practice in North Carolina (NC licensure or multi-state compact license required).

CCM (Certified Case Manager) Certification required within the first year of employment.

Preferred Qualifications :

Bachelors Degree in Nursing (BSN) or Masters Degree in a related field preferred.

ACMA (American Case Management Association) Certification preferred.
