AMG Home Healthcare Services is looking for qualified Physical Therapist interested in a part-time and / or per diem role to provide PT Home Health services to the Belle Glades community

Our Therapists will be responsible for evaluating, assessing, creating treatment plans, & treatment / progress follow-up with our Belle Glades pediatric and geriatric patients.

Responsibilities include :

• Evaluating and interviewing patients and their family members
• Creating individualized treatment plans that can be carried out at home
• Outlining treatment goals and methods to be used
• Assessing patients and monitoring their progress
• Coordinating with other healthcare professionals
