Job Description Summary
This position is for an experienced professional in hospice care, providing support and symptom management for terminally ill patients. The role requires strong critical thinking skills, comprehensive assessment capabilities, and effective communication for planning and conducting goals of care discussions. The candidate must be comfortable working in a variety of care settings including home hospice and inpatient units.
Experience and Requirements
- Minimum of 3 years of clinical experience in hospice care
- BLS certification is required
- Competence in managing complex symptoms associated with end-of-life care
- Capability in managing and administering medications through various routes including IV, subcutaneous, and oral
- Proficiency with narrative-heavy EMR systems necessitating excellent typing skills
- Strong skills in pain management and various nursing procedures such as central line care, tracheostomy management, wound and ostomy care, and symptom assessment
- Comfort with facilitating and participating in discussions about care goals and planning with patients, families, and teams
Patient Population
Primarily adults with terminal illness diagnoses, occasional pediatric cases in home settingsEquipment and Technology
Knowledge of specialized equipment including CADD pumps, feeding pumps, suction units, BiPap, CPAP, cough assist machines, nebulizers, IV pumps, glucose monitors, and COAGUCHECK for INR monitoringLaptops and work phones may be issued for patient management and communicationAdditional Information
Work primarily in designated geographic areas with occasional weekend coverage requiredAdaptability to different care environments within a hospice setting is expected.Reliable transportation is necessary due to the nature of home visitsSpecific scheduling details will be provided by the manager, and there is an expectation to work during major holidaysDress code for this position includes business casual or scrubs in non-institutional colors, with closed-toe supportive shoes. Personal grooming standards include tidy hair and limited jewelry.
