Job Title : Direct Support Professional – Per Diem II
Location : Rochester, NY 14624 – Various Locations
Department : Relief
Reports To : Workforce Management Coordinator
Employment Type : Per Diem – Minimum of 16 Hours per week required
Starting Wage : $19.50 / hr (Day Services) | $20.50 / hr (Residential)

Your Core Responsibilities :
Provide individualized support with daily living tasks including hygiene, meals, mobility, and medication administration.Encourage and support individuals in community activities, appointments, and recreational opportunities.Maintain safe and respectful environments while responding promptly to health and safety concerns.Follow individualized behavior support plans, nursing protocols, and safety procedures.Accurately document services, supports, and incidents in the Electronic Health Record (EHR).Maintain required certifications including SCIP-R, CPR / First Aid, Medication Administration, and Van Certification (as applicable).Demonstrate professionalism, compassion, and respectful communication with individuals, families, and staff.Pick up shifts flexibly through the agency scheduling app or Workforce Management Team.What You Bring :
Compassion, reliability, and a commitment to empowering others.Strong communication skills and the ability to work both independently and as part of a team.Flexibility to work at various sites and willingness to adapt to different program needs.Required certifications : SCIP-R, CPR / First Aid, and Medication Administration (training available if not already certified).Van Certification may be required depending on assignment.Ability to commit to at least 16 hours of availability per week.Completion of required background checks and clearances.Preferred : Prior experience in direct care or human services.Our Mission & Culture :
