Primary Care Physician Post Acute Care Central New York (1 hour from Syracuse)

Opportunity Highlights

  • Full-time position providing primary care in skilled nursing facility (SNF) settings
  • Outpatient post-acute care model with no hospital responsibilities
  • Flexibility to create your own rounding schedule (Monday Friday)
  • Collaborate with interdisciplinary teams and mentor nurse practitioners

  • Expected annual compensation : $183,000 $221,000 with uncapped productivity potential
