Florida Licensed Health Coach

A company is looking for a Health Coach (Cardiology) to provide patient education and support in a remote setting.

Key Responsibilities

Provide patient education on chronic disease management, particularly heart failure

Establish and monitor patient goals through shared decision-making

Collaborate with the clinical team to ensure optimal patient outcomes and maintain documentation

Required Qualifications, Training, and Education

Health coach certification from an accredited program; bachelor's degree in health sciences preferred

Experience in a medical office or healthcare environment; cardiology-related coaching is a plus

Experience with telehealth preferred

Proficiency in electronic health records and Microsoft Office applications

Bi-lingual fluency in English and Spanish highly preferred
