We are seeking a compassionate and skilled Nurse Practitioner to provide comprehensive primary care services to residents of assisted living facilities in Warren County, NY. The ideal candidate will have experience in geriatrics and / or long-term care and a strong commitment to delivering high-quality, patient-centered care.
In this role, you will conduct thorough health assessments, develop and implement individualized care plans in collaboration with interdisciplinary teams, manage chronic and acute illnesses, order and interpret diagnostic tests, prescribe treatments, and provide education and counseling to residents and their families. Job Requirements
Requirements
- Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program
- Unrestricted New York State Nurse Practitioner license
- Valid DEA permit to prescribe commonly used medications
- Eligible for Medicare and Medicaid billing and provider status
- Minimum of 3 years clinical experience, preferably in long-term care or geriatrics
- Experience with eClinicalWorks (eCW) electronic health records preferred
- Availability for full-time or part-time onsite work
