Community Health Integration Manager
Copley Hospital, located in Morrisville / Stowe VT, is looking for a Community Health Integration Manager to join the team! The Community Health Integration manager is a member of the Quality Department team tasked with supporting the development and implementation of organizational strategies to address population health goals through management of the Blueprint and Community Health Team / Initiatives.
This position is Full-Time (PT available).

Responsibilities
Essential Functions :
- Manage Community Health Team to implement population health strategies through integrated workflows.
- Implement structures to monitor status of team objectives i.e. team huddles, workplans.
- Managers Meetings, 1 : 1 check-in meeting with team members, and conduct reviews.
- Develop and implement strategies to address needs on the Community Health Needs Assessment and collaboration with community stakeholder's initiatives.
- Manage community partnerships for Copley to increase collaboration between primary care and community-based organizations.
- Champion community collaborative initiatives.
- Communicate about Community Health Team and population health programs to the community and partners.
- Establish referral protocols with community partners.
- Manage Blueprint projects in collaboration with Blueprint Facilitator. Attend Blueprint meetings as needed.

- Monitor / Manage Community Health Team budget
- Blueprint, grants, and other funding.
- Tracking expenses (training, travel, etc.).
- Co-develop strategies in alignment with clinical and quality initiatives to leverage ACO and other payor partnerships to support enhancement of care coordination and improve health outcomes.
- Leverage care coordination to meet quality measure goals for specific cohorts
- Support Copley Emergency Department (ED) utilization, inpatient admissions, hospital readmissions initiatives
- Manage new projects and develop implementation strategy for Community Health Team and Blueprint programs as needed
