The Center provides long-term skilled nursing care and short-term post-acute rehabilitation, including a ventilator unit. We are committed to the overall care of our residents through our culture, vision, and core values.

LPN Job Duties / Responsibilities

Provides nursing care to patients as assigned

Administers and records medications and treatments as assigned

Reinforce supports and provides health care instructions to patients and their families

Participates with other team members in planning, implementing and evaluating nursing care and other patient activities

Assists in the admission, transfer, and discharge of patients

Observes and reports patients' vital signs, symptoms and general condition to the appropriate individual(s)

Records on patient charts observation of patient’s responses to nursing care rendered

Maintains environmental conditions conducive to the comfort and safety of patients

Assists with unit management and / or assume charge of unit as assigned

Qualifications

Required Minimum Education

Graduate of an accredited Practical Nursing program

Required Minimum Experience

Minimum of 1 year nursing practice preferred; new grads are encouraged to apply.

Required Minimum Certification(s)

Current LPN license in the State of Georgia or NLC / eNLC Multistate License.

Basic Life Support (BLS)

Required Skills

Positive leadership, problem solving skills, strong organizational and time management skills

Strong assessment and clinical skills

Strong written and verbal communication skills. Ability to write and follow directions to resident's needs.

Use of Windows-based computer, utilizing electronic health records (EHR), email, internet, computer based learning modules, etc.
