  • week contract, 40 hours per week, day shift.
  • Key Responsibilities :
  • Provide occupational therapy in patients' homes.
  • Use Home Care Home Base for documentation.
  • Conduct start of care visits and manage a 25
  • point system.
  • Collaborate on patient care plans.
  • Qualifications :
  • CA Occupational Therapist license.
  • Basic Life Support (BLS) certification.
  • 2 years of home health or hospice experience.
  • Familiarity with Home Care Home Base.
  • Additional Information :
  • Overtime opportunities available.
