BAYADA Home Health Care is seeking a RN Case Manager f or our Allentown PA Pediatrics and Adults home care office . This office provides best in class care to clients throughout Allentown PA and surrounding areas.

Responsibilities for a RN Case Manager :

• Lead inspire and motivate others to provide exceptional care.
• Visit our Pediatric / Adult clients and conduct assessments in Allentown PA and other surrounding areas.
• Develop the care plan and oversee its execution.
• Supervise LPNs RNs and home health aides (HHA) staff in the field.
• Facilitate new hire orientation.
• Conduct competencies on new and tenured staff.
